SOFIA™ (Soft torqueable Optimized guide catheter for Intracranial Access) is a distal access catheter designed to navigate from the cervical internal carotid or vertebral artery into the intracranial circulation, placing a large-bore support platform close to the target lesion for coil, stent, and flow diverter delivery — or for direct aspiration thrombectomy in acute ischemic stroke. Its distal segment is soft and highly conformable, navigating tortuous intracranial anatomy without straightening the vessel, while the proximal shaft maintains sufficient pushability to advance the catheter to its target position. In the triaxial system standard for complex neuro procedures, SOFIA serves as the intermediate catheter between the guide catheter (or long sheath) and the microcatheter. Its 5F (0.055 in) and 6F (0.070 in) lumen sizes accommodate a Headway microcatheter and guidewire simultaneously, allowing coaxial device delivery. For stroke thrombectomy, the 6F SOFIA can perform direct aspiration (ADAPT technique) over a 0.014 in guidewire, or serve as the aspiration platform in a stent-retriever SOLUMBRA technique. SOFIA is available in 5F (DA5115ST at 115 cm, DA5125ST at 125 cm) and 6F (DA6115ST at 115 cm) with 17–19 cm distal soft segment and straight tip.

- ADAPT (A Direct Aspiration First Pass Technique) uses SOFIA as the primary thrombectomy tool in large-vessel occlusion stroke. The catheter is advanced over a 0.014 in neurovascular guidewire to the face of the clot, then aspiration is applied directly through the SOFIA lumen. The soft distal segment conforms to the clot face and the 0.055–0.070 in lumen creates suction sufficient to aspirate or engage the thrombus for retrieval without using a stent retriever.
- What microcatheters and wires are compatible with SOFIA™?
- SOFIA 5F (0.055 in ID) accepts a Headway 17 microcatheter with a 0.014 in guidewire coaxially. SOFIA 6F (0.070 in ID) accommodates a Headway 21 or Headway 27 microcatheter coaxially. Guidewire compatibility extends to all standard 0.014 in neurovascular wires, including the Traxcess family.
