FRED™ (Flow Re-Direction Endoluminal Device) is MicroVention's dual-layer flow-diverting stent for the endovascular treatment of large and giant cerebral aneurysms that are unsuitable for coiling alone. The device consists of a higher-density inner mesh that creates a flow-diverting metallic surface across the aneurysm neck — redirecting blood away from the sac and promoting progressive thrombosis and neointimal healing — mounted on a lower-density outer scaffold that provides radial force and allows flow through covered perforators and side branches. FRED's dual-layer architecture is engineered to decouple the competing requirements of flow diversion (high metallic surface area density) and branch preservation (low metallic coverage over perforators). The outer layer has sufficient open area for branch vessel flow while the inner layer concentrates coverage at the aneurysm neck. The device self-expands immediately upon release and opens predictably without the foreshortening variability that affects some competitive platforms. Deployment is made via the Headway 27 microcatheter. FRED is available in implant diameters from 3.5 mm to 5.5 mm, with working lengths from 7 mm to 58 mm depending on the target vessel diameter chosen for sizing. - What is the FRED™ flow diverter and what does it treat?
- FRED™ (Flow Re-Direction Endoluminal Device) is a dual-layer, self-expanding flow diverter for the endovascular treatment of large and giant intracranial aneurysms. It is placed in the parent artery spanning the aneurysm neck, redirecting blood flow away from the sac to cause progressive thrombosis and neointimal healing. FRED is particularly indicated for fusiform, wide-neck, and large saccular aneurysms where coiling alone achieves insufficient occlusion.
- What sizes does FRED™ come in?
- FRED™ is available in implant diameters from 3.5 mm to 5.5 mm (3.5, 4.0, 4.5, 5.0, 5.5 mm). Working lengths range from 7 mm to 58 mm depending on the target vessel diameter used for sizing. Catalog numbers run from FRED3507 (3.5 mm implant diameter, 7 mm working length) through FRED5526 (5.5 mm, 26 mm working length). All FRED devices are delivered via Headway 27 microcatheter.
- What is the difference between FRED™ and FRED™ X?
- FRED X adds X Technology — a nanoscale surface treatment covalently bonded to the nitinol strands — to reduce material thrombogenicity at the blood-device interface. FRED X also extends the size range downward to 2.5 mm (for distal/smaller vessel aneurysms) and features reduced delivery and retraction forces for improved navigability in tortuous anatomy. FRED X is the current-generation device; the original FRED is the established predecessor.
- What microcatheter is used to deliver FRED™?
- FRED™ (3.5–5.5 mm sizes) is delivered via the Headway 27 microcatheter (0.027 in inner diameter). FRED X in the smaller 2.5–3.0 mm System 21 variants is delivered via Headway 21 (0.021 in); the 3.5–5.5 mm System 27 variants via Headway 27. The delivery system requires a standard 6Fr guide catheter or long sheath.
- Does FRED™ require antiplatelet therapy?
- Yes. Like all flow diverters, FRED requires dual antiplatelet therapy (DAPT) — typically aspirin and clopidogrel or ticagrelor — started before the procedure and continued for a period defined by the treating physician. Antiplatelet protocols are physician-determined; FRED X’s X Technology is designed to reduce material thrombogenicity but does not eliminate the need for antiplatelet management.
