CASPER™ is a dual-layer carotid artery stent that addresses the most critical limitation of conventional single-layer carotid stents: the risk of embolic plaque debris release during and after deployment. CASPER pairs a standard outer nitinol stent scaffold — providing the radial force needed to stabilize the lesion and maintain vessel patency — with an inner micromesh layer of high-density nitinol. The inner mesh creates a fine-mesh liner inside the stent that acts as a physical barrier to plaque prolapse through the stent struts, reducing the rate of post-deployment debris release that has been associated with procedural stroke in carotid artery stenting. The dual-layer design gives CASPER a distinct advantage in patients with vulnerable, lipid-rich, or heavily calcified plaque where the risk of periprocedural embolization is highest — the same patients who are highest-risk candidates for carotid endarterectomy. Clinical data from the CRISTAL trial demonstrate that CASPER achieves low procedural stroke rates consistent with its embolic-protective mechanism. CASPER is available in diameters from 5 mm to 10 mm and dual-layer lengths from 16 mm to 40 mm, with a standard 0.014 in rapid-exchange delivery system. The overall implanted length is longer than the dual-layer segment to accommodate the transition zones at each end.

- What clinical evidence exists for CASPER™?
- CASPER has been evaluated in the prospective CRISTAL trial (Carotid stenting with dual-layer mesh CArotiD stent versus bare metal stent for treatment of carotid artery stenosis). The trial demonstrated low rates of periprocedural ipsilateral stroke with CASPER, consistent with the embolic protection mechanism of the inner micromesh. Multiple single-centre series also report CASPER's performance in high-risk surgical patients including post-radiation and restenotic carotid disease.
- Does CASPER™ require antiplatelet therapy?
- Yes. CASPER requires dual antiplatelet therapy (aspirin + clopidogrel or ticagrelor) started at least 3–5 days before the procedure and continued for at least 4 weeks post-procedure, transitioning to single antiplatelet long-term — the same DAPT protocol as other carotid artery stents.
